All offers for volunteer positions are contingent upon receipt of a successful child care criminal background screening. RYP will use the criminal history record solely for the purpose requested and cannot disseminate the record outside the receiving department, related agency, or other authorized entity. * I Acknowledge. By checking this box, I acknowledge that I have read and agree to RYP’s Background Check Information and Release document and give permission for RYP to perform a check of my background, including: criminal record, past employment, and volunteer history, educational and professional status and personal references. I understand that if I am over the age of 18, I will be responsible for paying $15 for my background check, and that I will not be allowed to begin service until a background check is complete. Criminal History*Have you ever been convicted of a crime other than a minor traffic violation?
